The statistics for boiler and pressure vessels safety are not encouraging for 1999. According to the National Board of Boiler and Pressure Vessel Inspectors 1999 Incident Report, accidents rose from 2011 in 1998 to 2163 in 1999, an increase of 7%. Injuries arising from these accidents increased from 31 in 1998 to a staggering 136 in 1999, an increase of 339%. Fatalities jumped from 9 in 1998 to 21 in 1999, an increase of 133%. Most of these incidents are due to OPERATOR ERROR OR POOR MAINTENANCE. As professionals we must take this seriously.
